This Privacy Notice is issued by Survive. This notice should be read in conjunction with Survive Privacy Policy. The Policy is displayed on Survive notice board and is freely available on request and on our website.

This notice is to inform clients, concerned others and staff how their personal information will be collected, used, stored and disclosed safely inline with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018 .

Inline with the Data Protection Regulations the lead Counsellor is the Data Controller for personal information processed by Survive. They can be contacted via Survives contact details above.

We collect personal information so that we can provide counselling services, ensure client safety, meet our legal obligations, manage referrals, communicate with clients, and improve our services.  Please Note Survive will not release information about you without your express permission, unless a safeguarding issue arises. ie a client was being abused, or in danger of harming themselves.

Survive collects personal information in the form of Name, address, Telephone numbers, email address and date of birth.

Survive also collects relevant details of your life history that you wish to disclose to enable Survive to carry out its function. 

Survive collects Personal Data and Sensitive Personal Data, also known as Special Category Data.

UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018, Special Category Data as Race, Ethnic Origin, Politics, Religion, Trade Union Membership, Genetics, Biometrics, Health, Sex Life and Sexual Orientation.

Survive will only collect information about your sexual life to the extent that you wish to disclose it. 

Survive will use your personal information for statistical purposes, reporting, funding applications, internal research and development. In all these cases Survive will only use statistical data.

Within GDPR there are six lawful bases for processing Survive has reviewed these and has identified that it will process data under 4 of the headers, Consent, Legitimate interests, Legal and Contract.

Consent – within this lawful basis the individual must have given clear consent for Survive to process their personal data for a specific purpose.

You have the right to refuse/withdraw consent at any time.

Legitimate interests – this process is necessary for Survive’s legitimate interests in order to deliver our services. This is used in instances where people’s data would reasonably be expected to be used for the purpose and has minimal privacy impact.

Legal – this processing is necessary for us to comply with the law

Contract – this processing is necessary for a contract we have with an individual, or because they have asked us to take specific steps before entering into a contract.

We will only share information without your consent where we have a legal or ethical duty to do so, such as where there are safeguarding concerns, concerns about serious harm to yourself or others, or where required by law. 

Survive stores and processes personal information securely within the United Kingdom wherever possible. Where trusted third-party providers, such as Google Workspace, are used to store or process information, appropriate safeguards are in place to ensure personal data is protected in accordance with UK GDPR. 

Survive takes appropriate technical and organisational measures to protect your personal information against unauthorised access, loss, misuse or disclosure.

Paper records are stored securely in locked cabinets within locked offices, with access restricted to authorised members of the Survive team.

Electronic records are stored securely on password-protected devices and within secure Google Workspace services, including Google Drive and Google Forms. Access is limited to authorised personnel who require the information to carry out their role.

Where we use trusted third-party providers, such as Google Workspace, they process information on our behalf in accordance with UK GDPR and appropriate security standards.

Survive will keep your personal data for 10 years after you have completed your therapy with Survive.

The data retained will be contact details, your numerical scores and who you worked with at Survive.

After 10 years Survive will safely destroy all your personal data records.

• Right of Access – individuals have the right of access to their personal data and supplementary information.

• Right to rectification – individuals are entitled to have personal data rectified if it is inaccurate or incomplete or is needed to be changed

• Right to erasure - individuals have the right to request the deletion or removal of their personal data where there is no compelling reason for its continued processing

• Right to restrict processing – individuals have the right to ‘block’ or suppress the processing of personal data. It can still be stored but not further processed.

• Right to data portability – the individuals are able to obtain and reuse their personal data for their own purposes across different services. This allows them to move, copy or transfer personal data easily from one IT environment to another in a safe and secure way.

• Right to object – individuals have the right to object to processing on legitimate interest of the performance of a task in the public interest, direct marketing and processing for purposes of scientific/historical research and statistics.

You can access your data by requesting it in writing from Survives data controller who can be accessed by the contact details above or through any member of Survive.
